Student Health and Immunization Requirements

Students attending university in North Carolina are required by law to present proof of certain vaccinations/immunizations in order to attend classes. You must submit the required Health Information and Immunization Form, by the due date noted on the form. 

All students are REQUIRED to show a record that they have been given the immunizations listed on page 4 of the form. Students who do not have a record of these immunizations will not be allowed to begin classes.

Process

  1. Take the Health Information and Immunization Form to your physician to fill out the appropriate dates you received each immunization and have them sign the form.
  2. Send your immunization records from a previous university or school as long as they are officially translated and signed.
  3. Send any officially translated record of immunizations from your physician or local health department and the completed Health Information and Immunization Form to hiif@wfu.edu by July 1 for programs beginning in the fall semester, or January 1 for programs beginning in the spring semester.

Students who do not bring a record of the required vaccines, or who have not received such vaccines, will be required to obtain those at the Student Health Center prior to being allowed to begin classes.

Student Health Insurance Requirements

Health insurance is required as a condition of enrollment for all full-time, degree-seeking students, as well as for all international students and visitors sponsored on an F-1 or J-1 visa by WFU.

Students enrolled in the LLM, SJD, or JD programs, as well as J-1 exchange students, must create a Student Blue account in order to request enrollment or waiver of the Student Health Insurance Program. 

Process

  1. Visit the webpage – www.bcbsnc.com/wfu (or https://studentblue.bcbsnc.com/wf/ if outside of the United States)
  2. Based on your election, you may choose – Enrollment > Enroll Now OR Waiver > Request A Waiver
  3. Create a Student Blue account as a NEW USER.
  4. Once the account is created, follow the prompts to complete the enrollment or application for waiver.

J-1 Visa Holders

Students participating in the Visiting International Researcher (VIR) program must also provide proof of health insurance as a requirement of the J-1 visa. The program that is recommended by WFU and meets the requirements of the visa is GeoBlue’s international health insurance plan. VIRs will be required to show proof of having purchased this plan, or another that meets the requirements, upon arrival in the US. For GeoBlue, VIRs should use the following group information to enroll:
